Your Right to Request

Eligible individuals can download a copy of their electronic 1095-C form through MyLandrum. In MyLandrum, click Employee Portal, Year-end Detail, then 1095-C. Eligible individuals are entitled to receive a paper copy of their Form 1095-C upon request. To request a paper copy of your tax form, please contact our team.
